الصدمة النفسية وعمل الحداد لدى الأم بعد الموت الفجائي لطفلها: دراسة في التحولات البنيوية تطبيق اختبار دراسة عيادية لحالتين بالمؤسسة الاستشفائية العمومية "شعبان حمدون" - مغنية - TAT

Abstract

تهدف هذه الدراسة إلى الكشف عن التحولات البنيوية التي تطرأ على الجهاز النفسي للأم إثر صدمة الموت الفجائي لطفلها. ولتحقيق ذلك، تم الاعتماد على منهج العيادي حيث تم دراسة حالتين من الأمهات، جرى اختيارهما بطريقة قصدية. وتكاملت في الدراسة مجموعة من الأدوات شملت: الملاحظة العيادية، المقابلة نصف الموجهة، مقياس اضطراب كرب ما بعد الصدمة لدافيدسون (PTSD)، واختبار تفهم الموضوع الإسقاطي (TAT). أسفرت النتائج عن وجود معاناة حادة من أعراض اضطراب ما بعد الصدمة مسببةً تصدعاً عميقاً في التنظيم البنيوي للجهاز النفسي. كما كشف التحليل الإسقاطي لاختبار (TAT) عن هشاشة واضحة في الأنا، واستنفار لدفاعات بدائية كالإسقاط والإنكار الجزئي، مع بروز حصر صدمي أمام اللوحات ذات الشحنة الانفعالية العالية. وأظهرت الدراسة وجود جرح نرجسي وتصدع في الهوية الأمومية تمثل في مشاعر الذنب والعجز، مما أدى بفعل فجائية الموت إلى تعطيل سيرورة الحداد الطبيعي وتطور حداد معقد جعل الأم عالقة بين استحضار الماضي والعجز عن استثمار المستقبل. وتوصي الدراسة بضرورة توفير تكفل نفسي عيادي متخصص يقوم على الاحتواء والمرافقة النفسية لإعادة بناء التوازن النفسي للأمهات المصدومات. Abstract This study aimed to uncover the structural transformations that occur within the mother's psychic apparatus following the trauma of her child's sudden death. To achieve this, the clinical method was adopted through a case study of two mothers, who were purposively selected. The study integrated a set of tools including: clinical observation, semi-structured interviews, the Davidson Trauma Scale (PTSD), and the projective Thematic Apperception Test (TAT). The findings revealed severe suffering from post-traumatic stress disorder symptoms, causing a profound disruption in the structural organization of the psychic apparatus. Furthermore, the projective analysis of the TAT highlighted a distinct ego fragility and the deployment of primitive defenses such as projection and partial denial, alongside the emergence of traumatic anxiety when facing highly emotional cards. The study also showed the presence of a narcissistic injury and a rupture in the maternal identity, manifested through feelings of guilt and helplessness. Due to the sudden nature of the death, the normal mourning process was disrupted, evolving into a complicated mourning that left the mother psychically stuck between invoking the past and an inability to invest in the future. The study recommends the necessity of providing specialized clinical psychological intervention based on containment and psychological accompaniment to reconstruct the psychic equilibrium of the traumatized mothers.
